Reports to:  Resource Navigator & Case Manager Supervisor

The Nevada Care Connection Case Manager provides person-centered case management to older adults, individuals with disabilities, and caregivers who face barriers in maintaining access to long-term supportive services. Responsibilities include conducting assessments, coordinating services, monitoring progress, and adjusting service plans based on changing needs. Case Managers also advocate for consumers to help them access and maintain essential programs and benefits, always supporting & honoring consumer choice.

The ideal candidate is compassionate, organized, and self-motivated, with strong communication and problem-solving skills.

Key Responsibilities:

  • Manage a caseload and provide ongoing case management support to consumers.
  • Conduct person-centered assessments to identify needs and develop individualized service plans.
  • Coordinate services and follow up to ensure referrals result in successful connections.
  • Conduct home visits within Clark, Esmeralda, Lincoln, and Nye counties, as needed.
  • Refer consumers to additional internal or external services as appropriate.
  • Provide direct and indirect advocacy to support access to services and benefits.
  • Maintain accurate and timely documentation in program databases such as Therap and Welligent.
  • Ensure compliance with confidentiality, HIPAA, and all program documentation standards.
  • Report consumer concerns, unmet needs, or service gaps to the supervisor in a timely manner.
  • Participate in the development or revision of program procedures to support efficient operations.
  • Attend relevant meetings and complete all required training as specified by ADSD Care Connection guidelines, including initial and annual training.
  • Assist with data collection for monthly, quarterly, and annual reporting.
  • Participate in outreach events, special projects, and other duties as assigned.

Education and Experience:

  • Bachelor’s degree in social services or related field preferred; equivalent experience considered.
  • Experience working with older adults required.
  • Experience with individuals with disabilities strongly preferred.
  • Knowledge of Clark County community resources required; rural resource familiarity a plus.

Skills & Abilities:

  • Strong verbal and written communication skills.
  • Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds
  • Excellent organizational skills, with the ability to manage time and tasks independently.
  • Proficient in Microsoft Word, Excel, Outlook; open to learning new systems.
  • Ability to maintain confidentiality and adhere to HIPAA and agency privacy standards.
  • Comfortable working both independently and as part of a collaborative team.
  • Ability to work the assigned schedule reliably and consistently.
  • Bilingual skills preferred but not required.
  • Must pass background check and drug screening.

Additional Requirements:

  • Valid Nevada driver’s license and reliable, insured transportation.
  • Willingness to travel throughout Clark County and surrounding rural counties.

Working Conditions:

  • Full-time position based in Las Vegas, NV.
  • Includes fieldwork and travel for home visits.
  • Must be able to sit, stand, walk, drive, and lift up to 10 lbs.
