Second Step Case Manager

The Second Step Intensive Case Manager will work directly with homeless clients to create a professional case management plan and perform related duties as required to meet program guidelines.

The Intensive Case Manager should be energetic, hardworking, and passionate about providing quality services to clients in crisis situations including the unsheltered. This position is dependent on grant funding. This job description may be changed as needed and without notice by management.

 Tasks/Responsibilities

  •          Carry a caseload of no more than 19 clients, with housing as the priority
  •          Conduct client assessment to determine client needs and set up a case plan to be followed.
  •          Provide in home intensive case management to program eligible individuals
  •          Complete all paperwork in a timely manner related to and for clients being assisted.
  •          Case Manger will provide appropriate resources, tools, counseling to assist participants in achieving their case plan goals. Making referrals for services, following up, tracking, and documenting participant’s progress.
  •         Maintain positive relations with clients, local community agencies and team members.         Document all client contact, maintain client files, and document all information       Be an active and collaborative member of the Emergency Services team at JFSA
  •        Participate in various committee meetings and training related to community resources.
  •         Maintain client confidentiality.
  •         Report client concerns/ needs to Director in a timely manner
  •          Maintain compliance with program guidelines.
  •          Perform other duties as assigned by the director and as needed for the programs.

Education and Experience

  •          Bachelor’s degree in social services or a related field preferred
  •          2 years of case management
  •          Experience working with the unsheltered
  •          Ability to communicate effectively verbally and in writing.
  •         Ability to work with individuals from various ethnic/cultural and socioeconomic backgrounds.
  •          Basic computer knowledge is required
  •          Possess a working knowledge of community resources within Clark County.

 Skills/Requirements

  • Dependable transportation and a current, valid, NV Driver License are required
  • Ability to pass a background test And Drug screening
  • Ability to work assigned schedule.
  • Ability to be empathetic while maintaining appropriate boundaries
  • Ability to work effectively and independently.
  • Ability to climb stairs, walk, drive, lift, and carry up to 25 lbs daily

Physical Demands: Ability to drive, sit, stand, and walk for extended periods of time.  Ability to perform push/pull movements and to lift 25 lbs.
