Senior Services Case Manager

The Senior Services Case Manager will perform professional case management with individuals 60 + years of age, to obtain health, financial and social services; conduct visits to clients' homes, provide assessments and develop and implement case management plans; performs related duties as required.

Jewish Family Service Agency Senior Services provides nondenominational, criteria-based social resources to Las Vegas seniors who need assistance in these areas: Transportation, Medical Support, Homemaking, Nutrition, and Transportation, with an emphasis on maintaining at-home independence and filling gaps in existing senior resources without duplicating services provided in the community. The Senior Services Case Manager should be energetic, hardworking and passionate about providing quality services to senior citizens within our community.

General Tasks/Responsibilities

  • Oversee assigned caseload.
  • Complete all paperwork in a timely manner related to clients being served.
  • Conduct client intakes to determine program eligibility.
  • Conduct visits to the client’s home to complete assessment and evaluations.
  • Refer clients to various community resources.
  • Maintain positive relations with clients, local community agencies and team members.
  • Participate in various committee meetings and trainings related to senior care issues.
  • Document all client contact, maintain client files and database information.
  • Assist in completing special projects and events set forth by JFSA.
  • Maintain client confidentiality.
  • Report client concerns/needs to Director in a timely manner.
  • Maintain compliance with program guidelines.
  • Assist in answering the phones.
  • Perform other duties as assigned by the Director.

Education and Experience:

  •  Bachelor’s degree in social services or a related field preferred.
  • Experience working with seniors is required.
  • Ability to communicate effectively verbally and in writing.
  • Possess a working knowledge of community resources within the Clark County area.
  • Ability to work with individuals from various ethnic/cultural and socioeconomic backgrounds.
  • Bilingual skills helpful.

Skills/Requirements:

  • Ability to pass a background check and drug screening.
  • Possess reliable transportation and a Nevada Driver’s License
  • Ability to work assigned work schedule
  • Ability to work effectively and independently.
  • Physical Demands: Ability to drive, sit, stand and walk for extended periods of time. Ability to perform push/pull movements and to lift 10 lbs.
  • Position duties will be fulfilled in an outdoor and indoor environment.

Required education:

  • Bachelor's

Required experience:

  • Conducting Home Visits: 1 year
  • Case Management: 1 year
  • Social Services: 1 year
  • Working with individuals 60+years old: 1 year

Required license or certification:

  • Driver's License
