On-Call Hospice Nurse

Job Description:
We are currently seeking a dedicated
and compassionate On-Call Hospice Nurse to join our team. In this role, you
will be responsible for providing high-quality end-of-life care to our patients
in their homes or facilities. As an On-Call Hospice Nurse, you will work
closely with our interdisciplinary team to ensure that our patients receive the
best possible care and support during this sensitive time.
Key Responsibilities:
- Conduct new admission assessments
for patients entering our hospice care program
- Coordinate and provide
comprehensive care for patients and their families according to their
individual care plans
- Perform same-day and bedside
documentation to ensure accurate and timely patient records
- Collaborate with physicians and
other healthcare professionals to optimize patient care
- Provide emotional support and
guidance to patients and their families
- Respond to patient and family
needs promptly and with empathy
-Answer phones and triage calls
appropriately
Qualifications:
- Registered Nurse (RN) license in
the state of Arizona
- Minimum of 2 years of nursing
experience, preferably in hospice or palliative care
- Strong assessment and critical
thinking skills
- Excellent communication and
interpersonal skills
- Ability to work independently and
as part of a team
- Willingness to work flexible
hours, including evenings, weekends, and holidays
- BLS certification required, ACLS
certification preferred
Schedule:
7 on 7 off, starting Friday 5:00pm.
