POSITION: Chaplain
REPORTS TO: Director of Nursing
JOB SUMMARY:
This position is a member of the Hospice Interdisciplinary Team and is responsible for the provision of spiritual support to, patients and families in keeping with the patient/family's desire and belief system in accordance with the plan of care and consistent with the mission, vision and goals of the Agency.
DUTIES AND RESPONSIBILITIES
· Acknowledge and respect the patients and family’s beliefs, culture and values related to life's meaning including suffering and loss, and desire for services/support.
· Assess the spiritual status of the patient, family and caregiver.
· Develop an individualized spiritual plan of care which demonstrates spiritual support as needed and defined by the on-going wishes and needs of the patient / family.
· Provide meditation, counseling, prayer, sacred rituals or practices, active listening and supportive presence, as appropriate.
· Work with other professionals in resolving spiritual / ethical issues.
· Document the spiritual assessment, goals for spiritual care, plan of care, care and services provided, and the patient's / family's response to the spiritual care.
· Assist with funerals and memorial services as requested by patient and/or family.
· Communicate with and support the involvement of local clergy and/or spiritual counselors as possible and as desired by the patient, family and caregiver.
· Attend Interdisciplinary Group meetings and participate in care plan update.
· Consult with and provide education to interdisciplinary team members, patients / families about spirituality related care and service.
· Work in a team approach with members of the interdisciplinary team to evaluate patient / family responses to and satisfaction with care.
· Upon request, meet with individual staff regarding personal spiritual issues that may affect their ability to function effectively.
· Provide bereavement support to staff.
· Participate in development of approaches to meet staff counseling needs.
· Acknowledge patient rights including privacy and confidentiality.
· Consistently display a professional appearance and conduct in all settings.
· Interact with customers using positive and professional approach.
· Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
· Support the mission, vision and goals of the Agency.
· Maintain professional growth and development.
· Participate in performance improvement activities.
· Abide by Agency policies and procedures.
· Accept other duties as assigned.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT
Work is mostly sedentary combined with moderate amounts of walking and standing. Must have visual and hearing acuity. Must be able to communicate clearly verbally. Must be able to read, write and speak English fluently. Attends and participates in activities off site as necessary. Must provide proof of automobile insurance upon hire and at each renewal and maintain a safe driving record.
EDUCATION, EXPERIENCE AND TRAINING
· Appropriate ministry certificate or chaplain/ministry experience.
· Ability to be open, flexible and ecumenical.
· Experience in working with patients/families dealing with life threatening illness and death.
