Reports to: Intake & Outreach Supervisor
The Intake Specialist
supports older adults, individuals with disabilities, caregivers, and those
planning for future long-term services and supports (LTSS) through
person-centered conversations that focus primarily on resource navigation. They
conduct brief assessments to understand consumer needs, identify appropriate
services, and guide individuals in accessing those services independently when
possible. Intake Specialists also provide limited information and referral, and
refer cases to Resource Navigators or Case Managers when consumers need more
intensive support to connect to or maintain access to services.
The ideal candidate is
compassionate, detail-oriented, and resourceful, with strong communication
skills and a commitment to supporting individuals in navigating complex systems
of care.
Key Responsibilities:
•
Conduct brief,
person-centered assessments to understand consumer needs, preferences, and
potential barriers to accessing services.
•
Provide guidance and
support to help consumers identify and connect with appropriate community
resources and services.
•
Refer cases to Resource
Navigators or Case Managers when consumers require more intensive or ongoing
support.
•
Document all consumer
interactions and services accurately and promptly in program databases such as
WellSky and Welligent.
•
Maintain compliance with
all data privacy, documentation, and program standards, including HIPAA.
•
Send relevant
documentation to consumers—including resource lists, privacy practices,
applications, and other supportive materials—in a timely and appropriate
manner.
•
Stay informed about
available programs, services, and public benefits across local and statewide
systems.
•
Collaborate with team
members to ensure effective service delivery and smooth internal referrals.
•
Participate in all
required training per ADSD Care Connection specifications, including annual and
program-specific trainings.
•
Support data collection
for monthly, quarterly, and annual reporting.
•
Assist with outreach,
events, and special projects as assigned.
•
Perform other duties as
assigned by supervisor or Director.
Education and
Experience:
•
High School Diploma or
equivalent required.
•
Bachelor’s degree in
social services or a related field preferred.
•
Experience working with
older adults is required.
•
Experience working with
individuals with disabilities is strongly preferred.
•
Familiarity with
community resources in Clark County is required; knowledge of rural resources
is a plus.
Skills and Abilities:
•
Strong verbal and
written communication skills.
•
Ability to work
effectively with individuals from diverse cultural, ethnic, and socioeconomic
backgrounds.
•
Excellent organizational
skills, with the ability to manage time and tasks independently.
•
Proficiency in Microsoft
Word, Excel, Outlook, and willingness to learn additional data systems.
•
Ability to maintain
confidentiality and adhere to HIPAA and agency privacy standards.
•
Comfortable working both
independently and as part of a collaborative team.
•
Ability to work the
assigned schedule reliably and consistently.
•
Bilingual skills are
helpful but not required.
•
Must be able to pass a
background check and drug screening.
Physical Demands: Ability to drive, sit, stand, and walk for
extended periods of time. Ability to perform push/pull movements and to lift 10
lbs.
